Online Questionnaire Phase

You will now be completing a series of questionnaires. All questionnaires should take approximately 30 minutes to complete. Be sure to put your participant ID at the top of every page in the appropriate box. Do not spend too much time on any one answer. Go with your gut response.

You will need the participant ID that was emailed to you. Please be sure that you have it handy before completing the study. You will need to enter it for each page of the online questionnaire portion.

Some of the questions in this survey are of a personal nature, but we assure you that your answers are completely confidential and that the data we are collecting are for research purposes only.
